Required Documentation

Immediately after the final rosters are published, parents for all players selected to participate for the Georgia Kings Basketball season must have all five (5) documents uploaded to DropBox prior to the first practice.

• BIRTH CERTIFICATE (copy)

• REPORT CARD for most recent period (copy)

• MEDICAL INSURANCE CARD (copy of front and back)

• PHYSICAL EXAMINATION (copy within past 12 months)

• PERMISSION FORM
